MHHOA General Scheduling Procedures:
Call for a monthly schedule
anywhere between the 20th through the 25th of each month for the following months schedule. I.E
call September 20 through 25 for an October schedule. Please have your personal schedule available so you can immediately
accept or decline game assignments while on the phone.

W-9’s and Independent Contractor agreements must be filled out and completed for all officials (If you
already completed and submitted these forms, you do not have to complete another). Scan the W-9 and email to the LIAHL Treasurer
Tom Duggan E-mail : tduggan@aaiinsurance.com and MHHOA Secretary Domenic Zecca domenic@alescavideo.com (The MHHOA secretary only gets the independent contractor agreement in addition to the W-9) you will not be paid unless s a W-9 is on file. You can get these forms on the www.liahl.com or the MHHOA website http://mhhoa.tripod.com/index/index.html. There are other forms that also
need to be completed and can be found on each of the websites noted. LIAHL assignments will be paid by the LIAHL (See additional
procedures below) all other games (not LIAHL) will be paid by the MHHOA.

If you must cancel a game assignment, you must do so as soon as possible
but in no case less than 3 days before your game. Once you accept an assignment it is your responsibility to make your assignments.
Canceling because you are sick is unacceptable; if you feel a cold coming on, cancel 3 days prior! You must inform the scheduler
of the name of your replacement. Any and all cancelations, substitutions or switching any games are subject to $2.00
cancellation fee, NO Exceptions!
- You will be
notified if your game is canceled due to weather or other unforeseen conditions. If you are not notified, the game is on and
you are to show up. If you show up and there is no game, you will be paid according to the MHHOA policy only if you notify
the scheduler.
- All game sheets
must be reviewed for accuracy prior to printing your name AND signing the score sheet. All game misconducts or any other ejections
must be noted clearly and distinctly on the score sheet. Keep a copy of the score sheet for your records, as you may need
to provide additional information and describe in detail any game misconduct. Report game misconducts to the appropriate parties
noted herein.
Additional procedures for Long Island
Hockey League Games:
- If you are assigned
an LIAHL game (played at Brewster, Ice Hutch or Rye Playland), you will be notified by the assignor as well as receive an
email notification from the LIAHL. You will receive email notifications from the LIAHL only for LIAHL league games.
You will not receive email notifications for any non-league LIAHL games. I.e. you may have received a game schedule indicating
you have games at 8am, 930am and 11am with only the 930am game being a LIAHL game. In that regard, you will receive and email
notification only for the 930am game, all other games are still being played, you just simply will not get an email notification.
- Your assignment
(either ref or linesman), game #, date and time will also be noted in the email. The referee is responsible for the game,
the score sheet, game reports if nec. and payment vouchers. Game payment vouchers are on the LIAHL website, you cannot
be paid unless a payment voucher and W-9 are submitted to the LIAHL. Make copies and keep them in your bag. Remember the
referee is responsible for submitting the vouchers in order for everyone (ref and both linesman) to be paid!! The vouchers
must be submitted within 1 week of the game(mail only) to Long Island Amateur Hockey League, Inc. C/O Rich Righi, 209 Michigan
Ave., Port Jefferson, NY 11777.
- Any game misconducts,
match penalties or any other game ejections must be reported to the LIAHL ref in chief, Joe Dicapua, dicapj@aol.com and to the LIAHL commissioner George Hinz at SharksSI@aol.com within 24 hours. See the LIAHL website for reporting procedures for game misconducts.
- You will be
notified if your game is canceled due to weather or other unforeseen conditions. If you are not notified, the game is on and
you are to show up. If you show up and there is no game, you still must complete all information indicate no game and officials
not notified. Print Mark D’Agostino on the game supervisor line and mail the voucher. You will be paid the full
game fee. You must also notify the MHHOA scheduler.
